An Air Force instructor pilot has died after the ejection seat of the plane he was in activated while on the ground at Sheppard Air Force Base in Wichita Falls, Texas.

The incident occurred Monday during ground operations. The instructor pilot was in a T-6A Texan II when the seat activated, causing the pilot to sustain fatal injuries.

The Air Force is investigating the incident to determine the cause of the ejection seat activation. The name of the pilot has not been released pending notification of family members.
